Supreme Dental operates two dental practices in Sydney, New South Wales: Supreme Dental Busby (Shop 2/103 Cartwright Avenue, Busby NSW 2168) and Supreme Dental Coogee (Level 1, 202 Coogee Bay Road, Coogee NSW 2034). Our principal dentist and founder is Dr. Anjali Chaudhari (BDS). Any reference to "Supreme Dental", "we", "us" or "our" in this policy refers to these practices and their staff.

Medicare and health fund details

Your Medicare number, private health insurance details (for HICAPS on-site claiming), and — where applicable — your Child Dental Benefits Schedule (CDBS) eligibility or NSW Oral Health Fee for Service Scheme (OHFFSS) voucher details.

Financial information

Payment records and, where relevant, partial payment card details used solely to process transactions.

Website and enquiry data

Information you submit through our online contact form, including your name, email, phone number, preferred clinic, and the nature of your enquiry.

3. How We Collect Your Information

We collect personal information directly from you wherever possible. This may occur when you:

We may also receive information from third parties with your consent — for example, from your GP, a referring dentist, Medicare, or your private health insurer.

4. How We Use Your Information

We use your personal information primarily to provide dental care and to manage your patient relationship with us. Specific uses include:

Providing and managing dental services

To plan, deliver, and document your treatment; to recall you for follow-up appointments; and to liaise with other health practitioners involved in your care.

Billing and Medicare claiming

To process payments, submit HICAPS claims on your behalf, bulk bill eligible children under CDBS, and process NSW OHFFSS vouchers.

Communication

To respond to your enquiries via phone, email, or SMS, and to send appointment reminders. We will only contact you for marketing purposes if you have given explicit consent, and you may opt out at any time.

Practice operations and safety

For internal administration, quality improvement, staff training, and to meet our legal and regulatory obligations as a registered health practice in New South Wales.

5. Disclosure to Third Parties

We do not sell, rent, or trade your personal information. We may share your information only in the following circumstances:

Treating health professionals

With your consent, we may refer information to other dentists, specialists, or medical practitioners involved in your care.

Government and regulatory bodies

To Medicare (for CDBS bulk billing), Services Australia, and NSW Health as required, or in response to lawful requests from courts or government agencies.

Health fund providers

To process your HICAPS claim or assist with private health insurance reimbursements.

Practice software and service providers

We use reputable, Australian-based dental practice management software and cloud services. These providers are bound by strict confidentiality and data security obligations.

Emergency situations

To your nominated emergency contact or to emergency services if necessary to protect your life or safety.

6. Storage and Security of Your Information

Your personal and health information is stored securely in our practice management system. We take reasonable steps to protect your information from misuse, interference, loss, unauthorised access, modification, or disclosure.

Paper records (where used) are kept in locked filing systems within our clinics. Electronic records are protected by access controls, password authentication, and regular security updates.

We retain patient records for the minimum periods required by law — generally a minimum of 7 years from the date of last service, or until a patient turns 25 (whichever is later), in line with NSW Health requirements.

7. Accessing and Correcting Your Information

You have the right to access the personal information we hold about you, and to request corrections if it is inaccurate, incomplete, or out of date.

To make an access or correction request, please contact us directly using the details below. We will respond within a reasonable timeframe. In some limited circumstances (e.g. where disclosure may affect another person's privacy or is restricted by law), we may be unable to provide full access, and will explain our reasons in writing.

8. Our Website

Our website at https://supreme-dental.com.au may collect standard technical information when you visit, including your device type, browser, IP address, and pages visited. This information is used only for analytics and to improve website performance — it is not linked to your identity.

Our website uses Google Maps to display our clinic locations. By using these features, you are subject to Google's Privacy Policy (https://policies.google.com/privacy). We do not control Google's data practices.
